Trip Cancellation and Interruption Insurance
We strongly recommend that your customer purchase comprehensive accident, medical, baggage and trip cancellation/interruption
insurance when space is reserved. Trip insurance will protect your customer from financial disappointment in the
event unforeseen circumstances preventing your customer from making their scheduled vacation. In the event it is
necessary to cancel or interrupt their vacation due to weather or any matter beyond the control of Caribbean Adventures/World
Dive Adventures and/or your chosen operator there will be no refund or credit issued. We highly recommend diving
accident insurance; please inquire with our reservation office for assistance. Caribbean Adventures does supply
Access America information. I have offered these options to my customer yes no. I understand it is my responsibility
to inform my customer of all above information.

Reservation, Payment, Cancellation, Refund and Airline Policies
INDIVIDUALS: A $__________ deposit per room is required to hold your reservation. A one hundred dollar per person
($100.00 pp) cancellation fee plus any applicable supplier imposed penalties is due when reservations are cancelled
more then 61 days prior to arrival. All reservations cancelled 31 to 61 days prior to arrival will forfeit your
deposit with no refunds given within 30 days of arrival.
GROUPS: A one thousand dollar ($1000.00) minimum deposit or three nights deposit per room is required with your
reservation. All group deposits are non-refundable.
PAYMENT: Full payment of your package is due 60 days prior to arrival. Full payment may be required 90 days prior
to arrival for certain holiday periods, specially priced packages or liveaboard dive cruises. Reservations made
within 30 days prior to arrival may be subject to a fifty dollar ($50.00) last minute booking fee. Any changes
to your reservations once confirmed may be subject to thirty-five dollar ($35.00) change fee.
TRAVEL AGENTS & DIVE SHOP PAYMENTS: A company check is required for all payments.
CREDIT CARD AUTHORIZATION FORMS: A credit card authorization form must be signed & returned to Caribbean Adventures/World
Dive Adventures prior to your reservation being confirmed.
PAYMENTS WITHIN 21 DAYS OF ARRIVAL: All monies paid within 15 days of arrival must be paid with a certified check,
money order or credit card. No space will be confirmed until payment has been received; all monies must be paid
in U.S. Currency.
AIRLINE INFORMATION: Airline tickets are non-refundable, and non-transferable. Restrictions and penalties apply
once ticketed. Price quotes for air travel are not guaranteed and are subject to changes until ticketed. Full passport
names must be given for any air travel. Incorrect names on tickets will not be the responsibility of Caribbean
Adventures/World Dive Adventures or its suppliers. Passports are required as proof of citizenship for travel. It
is your responsibility to reconfirm your flights. A $10 Federal Express service fee is added to your package when
shipping of airline tickets with documents is required. If you wish to waive this service please initial here__________.
Caribbean Adventures assumes no responsibility for lost or delayed tickets.

TERMS & CONDITIONS
Any deposit or payment to Caribbean Adventures Dive Travel Inc./ World Dive Adventures shows acceptance and acknowledgment
of the policies, terms and conditions as stated herein: Caribbean Adventures Dive Travel Inc./ World Dive Adventures,
herein after referred to as CA acts only as an agent for owners and contractors providing transportation, accommodations,
diving and other services. All tickets and vouchers are issued subject to terms and conditions under which transportation,
accommodations, diving and other services are offered or provided and constitute the sole contract between the
supplier and the tour member. CA shall not, at any time or in any way, be responsible for loss injury or damage
arising from an Act of God, Act of Government or defecto authority, war, civil unrest, any kind of hostilities,
strikes, thefts, sickness, quarantine, immigration or custom regulation, hijacking, breakdown, delay, cancellations,
error omissions or other cause beyond its control. Furthermore, CA reserves the right, without notice to make changes
or cancellations, in part or whole as circumstances dictate. CA accepts no liability for the consequences of the
personal activities or actions of any passenger who is deemed personally responsible for the decision concerning
conditions of health, safety, or fitness for diving and other activities, or any other circumstances beyond its
control.
Caribbean Adventures and its staff will not be responsible for, and will not issue refunds or credits for the following:
early departure by customer's choice; dives cancelled as a result of inclement weather, acts of God, Customers
failure to produce dive certification card: damages or costs resulting from postponed flight times, including but
not limited to missed transportation connections: nor for errors herein. Furthermore, CA will not be responsible
for loss of costs incurred as a result of passenger's failure to supply proper identification for immigrations
entry or participation requirements for diving or other activities. It is the passenger's responsibility to comply
with all customs and immigration laws.
