Please complete the authorization form in its entirety and please make note of the boxed authorization area offering a final payment option which will not be charged until your final due date when signed. Credit card numbers are not kept on file and can and will only be used solely for the transactions you have signed for on this form. All other transactions will require a new authorization form.
